The Brazilian playmaker, who is under contract with the German outfit until June 2011, has long been tipped for a move to the Serie A giants as a replacement for Pavel Nedved, who retires at the end of the season.A fee of around 25 million euros has already been agreed between the two clubs and the 24-year-old had a medical in Turin today.
"The transfer of Werder Bremen midfielder Diego to Italian top-flight side Juventus is soon to be complete," read a statement on the Bundesliga club's official website.
"The Brazilian was in northern Italy on Monday for a medical check-up."
Reports in Italy suggest Diego is set to pen a five-year deal at the Stadio Olimpico and will be officially unveiled at a press conference on Tuesday.
